Senior Care Managers identify social, financial and safety needs of senior citizens who wish to maintain independent living and assist in arranging and monitoring services. Enhanced Care Management is also available for older adults who receive Medi-cal. To be eligible for care management you must be 60 years of age and live in Santa Cruz County. If you would like to apply for the Care Management program please give them a call to schedule an intake appointment. They have Care Managers that speak English and Spanish and their office is wheelchair accessible.
