About Us
Health Projects Center supports people as they age to live safely at home by delivering high quality services and programs in the Monterey Bay Region. The offer the following programs for Seniors in Santa Cruz County:
AIDS Medi-Cal Wavier Program – (831) 459-6639
Support for Santa Cruz County residents living with HIV Disease and AIDS that have Medi-Cal. This program offers comprehensive care management and direct care services at no cost to the client. They are able to provide non-emergency medical transportation and have staff that speak English and Spanish. To be eligible for this program you must:
- Have a written diagnosis of HIV/AIDS with current symptoms related to HIV disease, AIDS, or HIV disease treatment;
- Have health status that is stable enough to make home care appropriate, and a home setting that is safe for the client and service providers;
- Have appropriate Medi-Cal eligibility and be certified to meet the Nursing Facility Level of Care or higher as defined in Title 22, California Code of Regulations, Sections 51334 and 51335.
- Must be 18 years of age or older.
They do not take walk-ins, please call to set up an appointment.
Del Mar Caregiver Resource Center – (800) 624-8304
The Resource Center works with caregivers of adults with neurological disorders such as Alzheimer’s Disease, Parkinson’s, Stroke, Traumatic Brain Injury and other conditions that may result in memory loss and confusion. They provide care planning, respite care, counseling and support groups, legal and financial consultation, information and referral, educational workshops and training. To be eligible for the Del Mar Caregiver Resource Center services the neurological disorders must have begun after they turned 18 and you must live in Monterey, Santa Cruz and San Benito County. There are amazing resources on their website for caregivers and information about other community partners. They have staff that speak English and Spanish and their website can be translated into Spanish. Their office is also wheelchair accessible.
MULTIPURPOSE SENIOR SERVICES PROGRAM (MSSP) – (831) 459-6639
MSSP is a free program that provides on-going care coordination, links participants to other needed community services and resources, coordinates with health care providers, and purchases some needed services that are not otherwise available to prevent or delay institutionalization. Other services they can provide are: minor home repair/maintenance, transportation, supplemental in-home chore, personal care, and protective supervision services, respite for caregivers, home delivered meals, and communication devices. To be eligible for this program you must be: 65 years of age or older, Medi-Cal eligibility, living in Santa Cruz County or Monterey County and willing to participate in an individualized care plan. MSSP has staff that speak English and Spanish and they also have access to translators to work with clients that speak other languages. Their office is also wheelchair accessible.
