Heartland Hospice serves residents of Santa Cruz County. Their services include in-home care, visits to hospitalized and contract nursing home patients, bereavement counseling and referrals to community resources. They accept Medicare, Medi-Cal, private insurance and private pay. To be eligible for services you must have had a Physician diagnose you with a terminal illness in the last six months or less. They have staff that speak English and Spanish and their office is wheelchair accessible.
